Not every cavity needs drilling. In some cases, tooth decay can be managed using a minimally invasive technique called SMART, which stands for Silver Modified Atraumatic Restorative Treatment.
SMART is designed to stop tooth decay while preserving as much healthy tooth structure as possible, making it an excellent option for many children, older adults, and anxious patients.
What is SMART?
SMART combines two proven treatments:
- Silver Diamine Fluoride (SDF) – a liquid that helps stop the bacteria causing tooth decay and hardens the softened tooth structure.
- Glass Ionomer Cement (GIC) – a tooth-coloured filling material that seals the cavity, restores the tooth, and slowly releases fluoride to help protect against future decay.
Together, these materials can arrest decay and restore the tooth without the need for extensive drilling in many cases.

- Often no need for local anaesthetic (needles).

Are There Any Disadvantages?
The main drawback is that Silver Diamine Fluoride permanently stains the decayed part of the tooth black. This staining is actually a sign that the decay has been successfully arrested, but it may not be suitable for highly visible front teeth.
SMART is also not appropriate for every cavity. Teeth with severe decay, infection, or nerve involvement may still require conventional fillings, root canal treatment, or extraction.
